The Information Technology Department located at the Catholic Education Centre (C.E.C) will provide students with Ontario Student Transcripts from July 5 to August 30, 2011. Requests for transcripts will be processed as follows:
1. Students may request transcripts by leaving a voicemail message at (905) 890-0708
ext. 20182.
2. A Transcript Request Form must be completed and be provided to reception upon pickup of the transcript. Copies of this form may also be obtained from reception and be completed on the day of pickup.
3. There are fees associated with the order and production of a Transcript.
· Fees only apply to those students who have graduated/withdrawn from a secondary school for over one year. However, a $5.00 fee applies for each additional copy.
· For former students, who have graduated/withdrawn from secondary school within the past five (5) years, the fee is $10.00 for the first copy and $5.00 for each additional copy.
· For former students, who have graduated/withdrawn from secondary school more than five years (where records are transferred to Archives), the fee is $20.00 for the first copy and $5.00 for each additional copy.
· The above fees are inclusive of regular postage. Any costs incurred for premium shipping (courier, express post, etc.) will be the responsibility of the applicant.
4. Acceptable Payment for Transcripts picked up at the C.E.C.
Cash, certified cheque or money order (payable to the Dufferin-Peel Catholic District School Board) is accepted. Please note there is no facility to pay by Visa, MasterCard or Debit at the C.E.C.
5. Every effort will be made to process transcripts in the following manner:
· Transcript requests received via the phone line prior to 9:30 a.m. will be processed and available for pickup at the C.E.C. after 1:30 p.m. that day.
· Requests received after 9:30 a.m. will be processed and available after 1:30 p.m. on the next working day.
· Please allow 3 business days to process Transcript Request Forms received via the (905) 890-7610 number.
6. Anyone picking up a transcript must show identification.
7. If someone other than the student is picking up the transcript, written authorization from the student is required, and the identification of the person picking up the transcript must be shown.
8. If the student is 18 years of age or over, a parent or guardian may not request or receive a transcript without written authorization from the student, (under the guidelines of the Freedom of Information and Protection of Privacy Act).
Any discrepancies in courses or marks will be resolved by the home school. Information Technology will not change any student mark or credit information.
If a student earns a diploma as a result of successful completion of a summer school course, the home school must update the student’s information in order to have the diploma information printed on the transcript.
Information Technology will only process requests during the time that the schools are closed.
