Student Transcript Requests

The Ontario Student Transcript is the official record of a student's academic achievement in the province of Ontario. Student records are confidential and transcripts are issued only on the written request of the student or parent/guardian (when student is under 18 years of age) and are subject to the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA).

During the school year, requests for transcripts are processed by the Guidance Department of the student’s current or last secondary school attended.

 

During the summer months, when schools are closed, requests for transcripts are processed at the Catholic Education Centre, 40 Matheson Boulevard West, Mississauga.

The Information Technology Department located at the Catholic Education Centre (CEC) will provide students with Ontario Student Transcripts between the hours of 8:30 am and 4:00 pm, from Monday July 11 to Monday August 29, inclusive.

Cost to Order a Transcript

There are fees associated with the order and production of a Transcript.

Fees only apply to those students who have graduated/withdrawn from a secondary school for over one year.  If you have graduated/withdrawn between Sept. 1 2010 and Aug. 31, 2011, there is no charge for the first copy. A $5.00 fee applies for each additional copy. 

    For former students who have graduated /withdrawn from secondary school within the past five (5) years, the fee is $10.00 for the first copy and $5.00 for each additional copy.  Please note:  This includes school years Sept. 2005 – Aug. 2006 to Sept. 2009 – Aug. 2010 inclusive.

    For former students who have graduated/withdrawn from a secondary school more than five years (where records are transferred to archives), the fee is $20.00 for the first copy and $5.00 for each individual copy.  This includes students who have graduated/withdrawn prior to Sept. 2, 2005.

    Transcripts can be mailed directly from the Catholic Education Centre (C.E.C.) and the above fees are inclusive of regular postage.

    The responsibility and costs for premium shipping, by courier, express post etc. will be that of the applicant.

    Acceptable methods of payment for transcripts ordered from the Dufferin-Peel CDSB include Cash, Certified Cheque or Money Order (payable to the Dufferin-Peel Catholic District School Board.  There is no facility to pay by Visa, MasterCard, American Express or Debit at the C.E.C.

How to Order a Transcript

Method 1:  This is the quickest, most reliable and preferred method.

Students may request transcripts by leaving a voicemail message at (905) 890-0708 ext. 20182. Please speak clearly and leave the following information:

    Date of your request

    Student name (clearly spell your first and last name)

    Student Identification or OEN number (if available)

    Name of person requesting if other than the student

    Last Dufferin-Peel secondary school attended

    Year of leaving school

    Date of birth

    Number of copies required

    Reason for the request

Who Can Order a Transcript?

    The student, the parent or the legal guardian if the student is under 18.

    If the student is 18 years of age or over, a parent or guardian may not request or receive a transcript without written authorization from the student as stipulated under the guidelines of the Freedom of Information and Protection of Privacy Act.

 When will the transcript be ready?

Every effort will be made to process transcripts in the following manner:

    Requests received via the phone line prior to 9:30 a.m. will be processed and available for pickup at the C.E.C. after 1:30 p.m. of the same day.

    Requests received after 9:30 a.m. will be processed and available after 1:30 p.m. on the next working day.

Please note:  We DO NOT call when your transcript is ready.  Just pick it up using the schedule above as your guideline.

 Payment Methods

Cash, certified cheque or Money Order payable to the Dufferin-Peel Catholic District School Board.  Please note: There is no facility to pay by Visa, MasterCard, American Express or Debit at the C.E.C.

 Who Can Pick Up a Transcript?

    If the student is under 18, the student, the parent or guardian can pick up a transcript but must show photo identification.

    If the student is 18 years of age or over, the student must show photo identification.  If the parent or guardian is picking up the transcript on behalf of the student, please bring written authorization to request as well as to receive the transcript.

At the time of pickup you will be given a Transcript Request Form to complete.

The Transcript Request form can also be obtained from the main Dufferin-Peel Catholic District Board website, http://www.dpcdsb.org/CEC.  Go to the Bulletin Board section and choose the button ‘Student Transcript Requests’ and click on the link Transcript Request Form.

Method 2: Via Mail or FAX, please allow at least two weeks to process a transcript.

Students may request transcripts by completing a Transcript Request Form and mailing or faxing the form along with a legible copy of photo identification.  The costs of ordering the Transcript are the same as those outline above.  Payment must accompany mailed requests.  Faxed requests will be processed when payment is received.  The mailing address and fax number are on the Transcript Request Form.

A Transcript Request Form may be obtained from the main Dufferin-Peel Catholic District Board website.  Go to the Bulletin Board section and choose the icon/button ‘Student Transcript Requests’.                                                                             

If you require your transcript to be faxed or mailed via regular mail from the Dufferin-Peel CDSB on your behalf, please indicate this on the Transcript Request Form and provide clear concise mailing or fax instructions.


Other Important Facts

Whether your transcript is requested via Method 1 or Method 2, the following points are applicable.

    Any discrepancies in courses or marks will be resolved by the home school.  The Information Communication and Technology Branch (ICT) will not change any mark or credit information.

    If a student earns a diploma as a result of successful completion of a course in an institution other than a Dufferin-Peel school, the student’s home school must update the information in order to have the diploma information printed on the transcript.

    For university and college applicants who left day school in June 2011, summer school marks and credits will be electronically transmitted and be available to OCAS and OUAC after August 13, 2011.

    ICT does not print Report Cards, Diplomas or release Ontario Education Numbers (OEN) via the phone.   The OEN is printed on your transcript if you were currently registered in a Dufferin-Peel CDSB school in the school year 2002-2003 or after.  If you require your OEN, please request a transcript via the procedures above.

    ICT will only process transcript requests during the time that the schools are closed.  If you require your transcript prior to Monday July 11, 2011 or after Monday August 29, 2011 inclusive, please contact your last Dufferin-Peel Catholic School attended.
