Frequently Asked Questions About Reporting Immunization Records
Peel Public Health is reviewing immunization records of all students in the Dufferin-Peel Catholic School Board. The Order of Suspension has been mailed out to all students that have incomplete records and suspension day is April 28, 2010.
To ensure that everyone has the required immunizations, Peel Public Health must have a record that shows that your child has had all the required infant/initial and booster doses of vaccine for measles, mumps, rubella, diphtheria, tetanus and polio (according to the Ontario Immunization Schedule).
When your child registered for school, you provided a copy of your child’s immunization record. The school sent this information to Peel Public Health. After registration parents are responsible to keep their child’s immunization record up to date with Peel Public Health. Your child’s record may still be incomplete if the record you submitted at registration did not show that your child has had all the required vaccines or if your child has become due for another vaccine since then.
Under the Immunization of School Pupils Act, the Health Department has the authority to suspend students from school if they do not meet the immunization requirements. Therefore students whose immunization records are incomplete will be suspended from school.
How to update your records with Peel Public Health
Update your records by using only one of the following ways:

§ Calling 905-799-7700 (Caledon residents can call free-of-charge at
905-584-2216)
Please make sure all faxed or mailed records clearly indicate the first and last name of the student, date of birth, school enrolled in and provide a phone number where you can be reached.
