Student Transcript Requests

The Ontario Student Transcript (OST) is the official record of a student's secondary school academic achievement in the province of Ontario. Student records are confidential and transcripts are issued only on the written request of the student or parent/guardian (when student is under 18 years of age) and are subject to the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA).

If the last secondary school you attended resided within the Dufferin-Peel Catholic District School Board and you require your transcript, please use the applicable process below to request your Ontario Student Transcript.

 

1. Applying to an Ontario College through the Ontario College Application Service (OCAS)

Please Note: This process is the same during the school year, including Christmas and March Break as well as July and August while schools are closed.

If you are a former student of the Dufferin-Peel Catholic District School Board and are applying to an Ontario college through the Ontario Application Service (OCAS) request your OST using the online electronic transcript request service available through OCAS.

To use this service you must:

·         Be a past student of the Dufferin-Peel Catholic District School Board

·         Apply to a minimum of one Ontario college

·         Pay the college application and transcript fees

To order your transcript online, go to ontariocolleges.ca

Please note:
If you are currently attending high school within the Dufferin-Peel Catholic District School Board, and are applying to Ontario colleges through OCAS, you do not need to request your transcript.  Your transcript information will be forwarded to OCAS on your behalf.  College applicants who left day school in June 2012, summer school marks and credits will be electronically transmitted and be available to OCAS after August 10, 2012 and from there be distributed to your selected colleges.


2. To Request your Transcript for all Other Purposes

During the school year:

 

From Monday August 27, 2012 to Monday July 8, 2013 requests for transcripts are processed by the Guidance Department of the student’s current or last secondary school attended. Please complete the Transcript Request Form and submit this form to the last secondary school you attended.


Please Note:  During Christmas Vacation and March Break the secondary schools are closed.  Please order your transcript from your last school attended prior to or after these times.

 

In Person:

Please submit the completed form and applicable transcript fee to the last secondary school you attended within the board.  You will be required to show identification when picking up your Transcript.  If your transcript request form is being submitted by someone other than yourself you must attach a photocopy of your identification to the completed transcript form.


By Mail or Fax:

Please mail the completed transcript request form with the applicable transcript fee (money order or certified cheque) and a legible photocopy of your identification directly to the last secondary school you attended within the board.  Click here to obtain the address and fax numbers of the secondary schools within the Dufferin-Peel Catholic District School Board. 

During the summer months:

From Monday July 9, 2012 to Friday August 24, 2012 inclusive and only when schools are closed, requests for transcripts are processed by the Information Technology Department (ICT) at the Catholic Education Centre (CEC), 40 Matheson Boulevard West, Mississauga.  Transcripts can be picked up between the hours of 8:30 am and 4:00 pm.  Transcript requests are only accepted and processed via phone request or by mail or fax.  Please follow the instructions outlined below.

 

By Phone:

Transcripts are to be requested by leaving a clear and concise voice mail message at (905) 890-0708 ext. 20182.  It is important that you leave the following information so that your transcript can be processed properly.  Incomplete information may result in a delay processing your transcript.

1.      Date of your Request.                                                    6.         Year of leaving school.

2.      Student Name (Clearly spell first and last name).        7.         Date of birth.

3.      Student Identification or OEN number if available.      8.         Number of copies required.

4.      Name of person requesting if other than the student. 9.         Reason for the request.

5.      Last secondary school attended.                                   10.       Contact Phone Number


By Mail or Fax:

When ordering by mail or fax, please allow at least two weeks to process a transcript.

Students may request transcripts by completing a Transcript Request Form and mailing or faxing the form along with a legible copy of photo identification.  The costs of ordering the Transcript are the same as those outlined above.  Payment must accompany mailed requests.  Faxed requests will be processed when payment is received.  The mailing address and fax number are on the Transcript Request Form.

If you require your transcript to be faxed or mailed via regular mail from the DPCDSB on your behalf, please indicate this on the Transcript Request Form and provide clear concise mailing or fax instructions


Who Can Request and Pick Up a Transcript?

If the student is under 18, the student, the parent or the legal guardian may order and pick up a transcript. Photo identification of the person picking up the transcript must be shown.

If the student is 18 years of age or over, the student must show photo identification.

 If the student is 18 years of age or over, a parent or guardian requesting the transcript on the student’s behalf must present written authorization from the student at the time of pickup. This written authorization must include permission to request as well as to receive the transcript. A legible copy of the student’s identification as well as identification of the person picking up the transcript must also be shown.

In all cases a Transcript Request Form must be completed.

At the time of pickup you will be given a Transcript Request Form to complete.

The Transcript Request form can also be obtained from the main Dufferin-Peel Catholic District Board website, www.dpcdsb.org.  Go to the Bulletin Board section and choose the button ‘Student Transcript Requests’ and click on the link Transcript Request Form.


When will the transcript be ready?

Please note:  We DO NOT call when your transcript is ready.  Just pick it up following this guideline.

Every effort will be made to process transcripts in the following manner:  Requests received via the phone line prior to 9:30 a.m. will be processed and available for pickup at the C.E.C. after 1:30 p.m. of the same day. Requests received after 9:30 a.m. will be processed and available after 1:30 p.m. on the next working day.


Where to pick up your transcript

Transcripts are to be picked up at the Reception Desk, between the hours of 8:30 a.m. and 4:30 p.m. Monday to Friday at the Catholic Education Centre, 40 Matheson Boulevard West, Mississauga (Map).

 

Cost to Order a Transcript

The following are fees associated with the order and production of a Transcript:

·         Graduated or left between September 1 2011 and August 31, 2012, inclusive there is no charge for the first copy. A $5.00 fee applies for each additional copy.

·         Graduated or left between September 1, 2007 and August 31, 2011 inclusive, the fee is $10.00 for the first copy and $5.00 for each additional copy.

·         Graduated or left prior to September 1, 2007 the fee is $20.00 for the first copy and $5.00 for each additional copy.

Transcripts can be mailed directly from the Catholic Education Centre (C.E.C.) and the above fees are inclusive of regular postage. Payment by certified cheque or money order payable to the Dufferin-Peel Catholic District School Board must be received prior to the Transcript being mailed.

The responsibility and costs for premium shipping, by courier, express post etc. will be that of the applicant.


Payment Methods

There is no facility to pay by Visa, MasterCard, American Express or Debit at the Catholic Education Centre (CEC). When picking up a transcript please pay with cash, certified cheque or money order payable to the Dufferin-Peel Catholic District School Board.


 

Other Important Facts

Any discrepancies in courses or marks will be resolved by the home school.  The Information Communication and Technology Department (ICT) will not change any mark or credit information.

 If a student earns a diploma as a result of successful completion of a summer school course, the home school must update the student’s information in order to have the diploma information printed on the transcript.

For university and college applicants who left day school in June 2012, summer school marks and credits will be electronically transmitted and we anticipate these marks will be available to OCAS and OUAC after August 10, 2012.

ICT does not print Report Cards and Ontario Education Numbers (OEN) are not released via the phone.  The OEN is printed on your transcript if you were currently registered in a Dufferin-Peel CDSB school in the school year 2002-2003 or after.  If you require your OEN, please request a transcript via the procedures above.

 ICT will only process transcript requests during the summer months when schools are closed.  If you require your transcript prior to Monday July 09, 2012 or after Monday August 27, 2012, please contact the last Dufferin-Peel Catholic School attended.
