The following forms must be completed:

• Registration Form
• Network User Application
• Media Consent Form
• Option Sheet
• Tax Form
• Profile sheet completed by Grade 8 teacher

REGISTRATION FOR NON-FEEDER SCHOOL STUDENTS:
(Grades 9 to 12)

The following forms must be completed prior to your interview with Iona’s administrative staff:

• Registration Form
• Network User Application
• Media Consent Form
• Option Sheet
• Profile Sheet by grade 8 teacher faxed to Iona (905-823-0248)
• Tax Form 
• Latest report card / transcript
• Proof of residency documentation (tax bill, telephone bill, etc.)
• Results of Literacy tests (grades 11 and 12)
• If from out of Peel Region, immunization records are required. 
• IEP documentation, if applicable
• IPRC documentation, if applicable

FOR ALL STUDENTS – VICE-PRINCIPAL MEETING:

(All students except feeder school grade 9 students)

Our vice-principals are allocated alphabetically, according to surname. 

Please contact the appropriate vice-principal to arrange for an interview.
