Individuals who are not fluent in English should be accompanied by a translator.Individuals may be asked to provide proof of address, such as a photo ID (driver’s license, student card) bill, OHIP card, etc or proof of work in Ontario. Individuals 14 years of age or older may sign their own consent. Parents/legal guardian signature is required for those under 14 years.
