• Baptismal certificates for child and one parent
• Child’s birth certificate
• Immunization record
• Immigration papers (if applicable)
• Proof of address

For a student who is currently  in a Dufferin Peel School

• Baptismal certificates for child and one parent
• Child’s birth certificate
• Proof of address
• Student transfer form from previous school
