Registration Procedures

Students new to Our Lady of Mercy School are required to register with the following documents:
• Proof of age (Birth Certificate)
• Immunization Record
• Original Catholic Baptismal Certificate of the Roman, Greek or Ukrainian Rite for one custodial parent and child

•Proof of Canadian Citizenship or Landed Immigrant Status

• Proof of address (utility bill, bank or credit card statement)

We request that you provide original documentation  in order to facilitate your child’s registration.  We will photocopy your original documents and return them to you.
