Student Medical Information
In order to ensure the health and safety of our students, it is imperative that our school is aware of any medical conditions in order to be prepared in the event of an emergency. Kindly advise the main office if your child has any medical needs. Specific documentation ("GF514") is needed for the following conditions: Diabetes, Epilepsy or other seizure disorder, Asthma or Anaphylaxis. Please see the links below and complete the appropriate form(s).
NOTE: Completion of any GF514 form(s) is only needed for those who have not yet previously submitted it to the IONA school team, or if changes need to be made to a previously submitted form.
