If your child suffers from the following medical conditions: Anaphylaxis, Asthma, Epilepsy, or Diabetes, please complete the appropriate form below and email it to:

 If your child has an existing medical condition and you have already completed medical forms for him/her at St. Michael's, you are only required to complete the GF035 form. This form does NOT require a physician's signature if there are no changes to your child's condition/medication.



Students or parents and legal guardians can check if their child has already received their routine school immunizations through the Immunize Connect Ontario (ICON) website or by asking their primary care provider. Meningococcal vaccine is a single dose. HPV and Hep B are a two- or three-dose vaccine series, depending on age at time of vaccination. 


By getting these vaccines, students will be protected against serious diseases, including preventing cancer later in life.

eLearning 2022-23 

Ministry of Education Update for eLearning:On February 1, 2022 the Ministry of Education issued the Policy and Procedures Memorandum (PPM 167) regarding the Online learning graduation requirement. 
Beginning with the cohort of students who entered Grade9 in the 2020-21 school year, all students must earn a minimum of two online learning credits as part of therequirements for an Ontario Secondary School Diplomaunless they have been opted out or exempted in accordance with the processes described in the memorandum (PPM167).Adult learners entering the Ontario secondary school system in 2023-24 or later will also be required to meet this graduation requirement unless they opt themselves out of the requirement in accordance with the processes described in thismemorandum. The complete memorandum can be accessed in the link provided herein, for your perusal. Policy/Program Memorandum 167

​SafeArrival Absence Reporting Tool

A new absence reporting tool, SafeArrival, will be launching at all Dufferin-Peel schools on Monday, May 3rd.  SafeArrival is an application within SchoolMessenger that will allow parents/guardians to report their children’s absences in one of the following three ways:

What is Netiquette?

Netiquette is short for "Internet etiquette."  It is a code of conduct for good and appropriate behaviour on the internet.  Similar to etiquette it is a respectful way of conducting oneself while engaging with others online.
